Other necessary forms that students might encounter on a case-by-case scenario are included in the Graduate School website which includes information and forms on transfer credit, thesis, scholarships, fellowships, assistantships, and grants.

Academic Forms
- Independent Study (ECO 400): To be completed by the student and the student’s direct faculty supervisor and submitted to the graduate program director.
- Final Exam Conflict: Verification of three or more final exams on one day. To be submitted with the University Registrar.
- Professional Practice (ECO 498): Students who apply for an internship must also complete the Professional Practice Health Insurance Certification form.

Employment Forms
- USCIS Form I-9: Employment eligibility verification form from the Department of Homeland Security. Read more about the I-9 form. Required document for a Graduate Assistant position before the time of first employment at the University. New domestic students can file the form at the Department of Economics. New international students must file the form in the Human Resources office.
- USCIS Form I-765: Application for employment authorization from the Department of Homeland Security. Read more about the I-765 form. Required document for a Graduate Assistant position before the time of first employment at the University. New international students must file the form in the Human Resources office.
- Social Security Number and Card: A Social Security number is important because you need it to get a job, collect Social Security benefits and get some other government services. You should keep your Social Security card in a safe place with your other important papers and avoid giving it out unnecessarily. - Graduate Assistant Student Health Insurance Certification: Required document for a Graduate Assistant position to be submitted every semester to the Department of Economics.

Graduation Forms
- Master’s Degree Audit: Submit the form at the beginning of the semester before your planned semester of graduation to the graduate program director, Dr. Nikolaou. Detailed due dates for each semester can be found at the Graduate School website.
- Graduation Application: Submit the online application and the graduation fee at the start of the semester before your planned semester of graduation
- Request to Extend Time to Degree: Only for master’s students whose six year to degree has expired. To be submitted to the graduate program director.

- ISU Symposium or Presentation Assistance Grant: Application to be reimbursed for expenses incurred for participation in the annual University Research Symposium.
- Three Minute Thesis: Advisor consent form for participation in the Three Minute Thesis competition.

Tuition Waiver Forms
- Graduate Tuition Waiver: Application for tuition waiver to be submitted to the graduate program director of the Department of Economics, Dr. Nikolaou.
- Diversity Tuition Waiver: Application for tuition waiver awarded competitively to U.S. citizens from traditionally underrepresented groups. To be submitted in person at the Graduate School.
